I'm not saying it can't be done but I won't do it.
What you're asking is to transfer a service from your hosting provider to a script you can use on each server perl is installed on. What I advice you is to search for a perl or php script that grabs all information from a form and sends it to your emailaddress. There should be some of them on the internet.
For example you can look at nms which doesn't do the above but it lets you create highly costumized mailforms, The perl archive or just search with Google
