 Internet Quiz Construction Script

Hi everyone- I'm in search of an online-quiz construction software I can use for my webpage. If I could find some script that I could customize and add result images and messages, that would be great. The questions are both "preference"(What is your favorite feature of my site?) and "correct answer"(What is the capital of Florida?) types. I've searched high and low, but I can't seem to locate a script that will allow the different types of questions and display result images/paragraphs. If you know of any script that could help me please let me know!
