Need help with script

Ok, I'm famous for it. Bunging up cgi scripts real good. I admit.
I'm hoping someone can help me here.
I have a great little classifieds script (1 file) and I've been trying
to edit the header and left-side navigation to the other categories. The
problem is, every time I've tried to add the left hand navigation and/or
headers, the script doesn't want to post ads anymore.

I just can't seem to figure out how to insert the header and left
navigation without causing the errors and I've tried everything!
Any help/advice is appreciated.
