A script to handle RSVP

I need a script that will allow a visitor to come to a form and input four fields. (name, number attending, number not attending, comments) I then need for the results to be written to a password protected html page that displays the total number attending, the total number not attending, as well as list of the names of people attending with the number in their party that are attending, a list of those not attending with the number in their party not attending along with whatever comments. The script needs to be able to handle RSVP's for multiple events - with each event being written to a different page.
