Looking for "bad links" CGI script like one used here

Hello all. I have tried to find this script on this site but there's so much going on here I may have missed it. Then again, maybe it's not available? I'd like to know where I can find a Perl script that will allow a visitor to report a bad link (in my case, download links -- literally thousands of them on my site) like the one used here. I like the fact that a visitor doesn't have to fill out a form. One click and a click to confirm and it's reported. Perfect!! I'm combining two of my sites from two servers into one big site and there are so many download links and files attached thereto, that it's possible I'll miss some or miscode others. If I do, I'd like a system whereby the visitor can report that the download link doesn't work without me having to give out my email address and without the visitor having to complete a form. Is such a script available on this site? If not, ideas on where I might find a similar one? I'd prefer that it be freely available but am willing to look into just about any possibility.
