generating html on the fly which includes an image and form

I'm looking for a script that when a user is viewing an image, they could click on "buy print" and the script would generate an html page on the fly that would include the previous image as well as a standard form to fill out.
An example of what I'm trying to do: http://www.danheller.com/images/Bestof/img25.html
(He just has one step between, I have the buy print and license print on that first page.)
