Auto Fill Form Fields

Is there a script that has the ability to automatically fill in form fields with a user's info.
What I need to be able to do is fill in the NAME and EMAIL ADDRESS field on a form automatically with the info of the person who is using the form.
Thanks in advance.
PS: a great example of something like this is used RIGHT HERE IN THE PERL ARCHIVE FORUM...
When I selected the MAKE A NEW POST icon I am taken to a form where my username is ALREADY filled in. That is what I want to be able to do.
