File Upload and Autolink

I am looking for a script that can upload files from within the browser (I am aware that there are many that can do this), but then I want it to automatically generate links to the uploaded files. Ideally so that I would only have to do something like to add the links to a page.
If possible, I would also like to be able to add the name I want for the link so that it is not just the filename.
If anyone knows of a program that does this, or is keen to write/modify one, I would be forever grateful.
